"As the data will indicate the main objectives of the project were realized by most of the participants. They gained information about their diagnosis, learned about the medications they taking and made the decision to continue with the medication until having a discussion with the provider about whether to stop or change amounts. They also began inquiring about why test or procedures were given and what they were to do.  In essence we saw patients become empowered to participate in their health care through the use of the planner."
