"Through the project we were able to convince participants that by using the planner to record important aspects of their health care and not leaving a medical visit until they understood their medical diagnosis, they could improve their health. "As the data will indicate the main objectives of the project were realized by most of the participants. They gained information about their diagnosis,pharmacy online ,learned about the medications they taking and made the decision to continue with the medication until having a discussion with the provider about whether to stop or change amounts. They also began inquiring about why test or procedures were given and what they were to do. In essence we saw patients become empowered to participate in their healthcare through the use of the planner."
